HOW WE WILL MEET THE PRINCIPLES OF THE GDPR

We will  process your personal information fairly and lawfully by;

a) Only collecting and using your information to provide you with your care and treatment and will not use it for anything else that is not considered by law to be for this purpose;

c) Only using enough of your personal information that will be relevant and necessary for us to carry out various tasks within the delivery of your care;

d) Keeping your information accurate and up to date when using it and if it is found to be wrong, we will make it right, where appropriate, as soon as we can;

e) Only keeping your information in a way that it will identify you for as long as we are legally required to, whilst ensuring your rights;

f) Having secure processes in place to keep your personal information safe when it is being used, shared, and when it is being stored.

WHAT INFORMATION WE WILL COLLECT FROM YOU

Athena Holistics Ltd will keep records about your health and any treatment you receive.  This will include:

Basic details such as name, address, date of birth, phone number, and email address  – where you have provided it to enable us to communicate with you by email
Your next of kin and contact details
Notes and reports about your physical or mental health and any treatment, care or support you need and receive
Information on medicines, side effects and allergies
Patient experience feedback and treatment outcome information you provide

Your treatment records are manual and will be stored in a locked filing cabinet in a locked, secure room.

Your basic details (see above) will be held on a computer and secured with a password.

HOW LONG DO WE KEEP YOUR INFORMATION?

Your treatment records remain the property of Athena Holistics Ltd and will be retained indefinitely, for a minimum of 7 years from the date of your last treatment.

As a client you can, by written application, seek access to these notes as per the Data Protection Act 1998. Records of such requests will be kept on our files.

In some cases records may be required by solicitors pursuing accident insurance claims. Your records will not be made available unless we have written permission from yourselves.
In the event of the practice being sold or transferred to another health professional, your original records will only be passed on if (a) the new owner is subject to the policy outlined above and (b) the patient is informed in writing in advance of the transfer and given the opportunity to object, in which event Athena Holistics Ltd will retain the original records.
You will be kept fully informed and offered appropriate choices about continuing care and the safe keeping and location of your original records.
